The Vegetarian Athlete's Iron Deficiency Problem

A 21-year-old state-level runner came to us earlier this year with a resting heart rate that had crept up ten beats in six weeks, sessions that felt “heavier than they should,” and a ferritin reading of 14 ng/mL. She'd been vegetarian her whole life. Nothing had changed in her training. What had changed was that her iron stores had finally run out.
This is the pattern we see most often with vegetarian and vegan athletes, and it rarely announces itself. Iron deficiency builds quietly for months before it shows up as fatigue, breathlessness on climbs, or a plateau that no amount of extra training fixes.
Quick Overview
The gap is real, but it's about absorption, not quantity.
Vegetarian athletes often eat as much total iron as omnivores, sometimes more. The problem sits in the type of iron, not the amount. Plant foods carry non-heme iron, which the gut absorbs at roughly 2–20% depending on what else is on the plate, against 15–35% for the heme iron in meat. A 2021 analysis of 1,340 individuals found vegetarians running measurably lower ferritin than omnivores across age and sex groups (Slywitch et al., Nutrients, 2021).
Phytates in grains and legumes, tannins in tea, and calcium in dairy all block non-heme absorption further. None of these foods need to disappear from a vegetarian athlete's plate. They need to stop sitting next to the iron source at the same meal.

Training makes the iron deficiency worse, not neutral.
Endurance athletes lose iron through mechanisms that have nothing to do with diet. Every footstrike in running ruptures red blood cells in the capillary beds of the feet, a process called foot-strike haemolysis. Iron is also lost through sweat and gut microbleeding during long sessions.
The bigger driver is hormonal. Hepcidin, the liver hormone that controls how much iron the gut absorbs, spikes after hard training and stays elevated for hours. In trained runners, a single prolonged session raised hepcidin enough to cut fractional iron absorption by over a third for the rest of that day (Peeling et al., PLOS ONE, 2014). An athlete who trains daily is fighting a body that actively blocks iron uptake on the days it needs it most.
This is why a vegetarian distance runner or a vegan strength athlete training six days a week needs a materially different iron strategy than a sedentary vegetarian. The training load isn't incidental to the problem. It's a second cause layered on top of the first.
Ferritin tells you what haemoglobin can't
Most athletes only get a hemoglobin test, which stays normal until iron stores are almost completely depleted. Ferritin reflects stored iron and drops long before hemoglobin does. I want to see ferritin above 30 ng/mL in a female athlete and above 50 ng/mL in a male athlete before I call iron status adequate for training. Below 20 ng/mL, performance is already compromised even if the athlete hasn't noticed yet.
I also run this test twice a year for any vegetarian or vegan athlete in consistent training, not just when symptoms show up. Fatigue that gets blamed on training load, sleep, or stress is iron deficiency until proven otherwise in this population.
Closing the gap without overcorrecting
The fix isn't just eating more iron-rich plants. It's pairing them correctly. Vitamin C is the single most effective lever available: it converts non-heme iron into a form the gut absorbs far more efficiently, and it can counteract the inhibitory effect of phytates and calcium in the same meal (Cook & Reddy, American Journal of Clinical Nutrition, 2001). A lentil dal with a squeeze of lemon, or spinach with tomatoes, does more for iron status than the same food eaten alone.
Timing matters as much as pairing. Tea, coffee, and milk should sit at least an hour away from an iron-rich meal or any iron supplement, since they block absorption when taken together. For any vegetarian or vegan athlete I put on supplemental iron, vitamin C goes alongside it every time, and I don't run supplemental iron without a blood test confirming it's needed first. Iron overload carries its own risks, and self-dosing without testing is a bad trade (Nolte et al., German Journal of Sports Medicine, 2024).

We don't tell vegetarian athletes to add meat, and we don't tell them iron deficiency is inevitable on a plant-based diet. Both are lazy answers.
What we do is test ferritin twice a year, build every iron-rich meal with a vitamin C partner already on the plate, and keep tea and coffee away from meals that are meant to be doing iron work. That's the entire protocol, and it's the difference between an athlete who plateaus quietly for months and one who never has to find out what a ferritin of 14 feels like mid-season.
